Originally founded to provide evidence based  implementation tools to the referral physicians of the Ottawa Cardiovascular Centre and the CONDOR Network, Continuing Medical Implementation Inc. strives to make these invaluable tools and information sheets available to a broader spectrum of physicians.

Guide for Comprehensive Risk Reduction. This core document provides the busy practitioner with a complete and up to date checklist to guide the optimal management of all aspects of CV Risk. Coupled with the CV Risk Flowsheet these tools provide a virtual cardiac prevention clinic which you can download, print, copy and incorporate into your practice.

Guide for Heart Failure Management. This core document provides a concise and up to date summary of current state of the art CHF management. Coupled with the HF Flowsheet these tools provide a virtual heart failure clinic which you can download, print, copy and incorporate into your practice.

Lipid Optimization Tool. This core document provides the user with a risk stratification scheme, lipid therapeutic targets, % LDL reduction table and statin and fibrate dose response chart as well as a proven therapeutic strategy that will expedite and document optimal lipid management.

The Cardiovascular Risk Reduction Toolbox (cvtoolbox) provides you the physician with the resources required to optimize cardiovascular risk in your patients.

 

A Guide for Cardiac Rehabilitation (cardiac rehab). The purpose of this booklet is to provide the patient with the information about heart disease and recovery after suffering a heart attack.

Heart Failure Survival Kit (hf survival kit) is a compendium of do's and don't's in kit form for patients with congestive heart failure, that suggest specific diet lifestyle and exercise changes of benefit in heart failure. As well as the kit provides information on current heart failure management.
